Ohio Death Certificate

Qualifications

There are no restrictions on who may request certified death certificates in Ohio.

Cost

The fee to search for a death certificate is $21.50, which includes one certified copy of the death certificate. Each additional copy of the record ordered at the same time is also $21.50. Checks or money orders should be made payable to the "Treasurer, State of Ohio." Cash is not accepted, except if the request is made in person. Fees are not refundable.

If requesting the certificate from the Vital Statistics office in the county in which the death occurred, application fees may vary. Contact the county for information on their fees. For a list of Local and Deputy Registrar’s of Ohio Vital Statistics offices, click here.

You may apply in person at the Ohio Department of Health Office of Vital Statistics, located at:

The office is open from 8:00 am until 5:00 pm, Monday through Friday, and closed holidays. Any walk-in after 4:30 pm is not guaranteed same-day service; an order can be dropped off and scheduled for pick up or staff will mail it out the next day.
