Nevada Death Certificate

Below is the information you'll need in order to obtain a death certificate in Nevada, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

The following people may request death certificates from the state of Nevada:

  • Direct family member by blood or marriage
  • Legal guardian
  • Legal representative

Cost

The fee to search for a death certificate is $25.00 per certificate for Carson, Clark, Douglas, Lyon, Mineral and Washoe counties, and $22.00 per certificate for all other counties. This includes one certified copy of the death certificate. Each additional copy of the record is also either $25.00 or $22.00 depending on your county. Checks or money orders should be made payable to the "Office of Vital Records." Cash is not accepted. Fees are not refundable.

  • Father's name
  • Mother's maiden name
  • Funeral home
  • Your relationship to the deceased
  • Your legal need for the death certificate
  • Your name
  • Your mailing address
  • Photocopy of current, government-issued photo ID
