Maine Death Certificate
Below is the information you'll need in order to obtain a death certificate in Maine, including qualifications, cost, required information, and where to submit your request.
Qualifications
The following people may request death certificates from the state of Maine:
- Spouse or registered domestic partner
- Parents
- Descendants of the deceased (including children, grandchildren, and great grandchildren to the most remote degree)
- The legal custodian, guardian, or authorized representative of the deceased
- Genealogists who have a researcher card issued by the Maine Center for Disease Control and Prevention, Office of Data, Research and Vital Statistics
Cost
The fee to search for a death certificate is $15.00, which includes one certified copy of the death certificate. For each additional copy of the certificate ordered at the same time, the fee is $6.00. Checks or money orders should be made payable to "Treasurer—State of Maine." Cash is not accepted, except if the request is made in person.
Required Information
- Name of the decedent
- Date of death
- Name of city/town in which the death occurred
- Your relationship to the name on the death record being requested
- A daytime telephone number
- Photocopy of current, government-issued photo ID
- A long, self-addressed stamped envelope
