Louisiana Death Certificate
Below is the information you'll need in order to obtain a death certificate in Louisiana, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.
Qualifications
The following people may request death certificates from the state of Louisiana:
- Surviving spouse
- Parent
- Adult child
- Sibling
- Grandparent
- Adult grandchild
- Legal guardian
- Beneficiary of an insurance policy of the deceased
Cost
The fee to search for a death certificate is $9.00, which includes one certified copy of the death certificate. Each additional copy of the record is also $9.00. A $0.50 state charge will be added to each mail order. Checks or money orders should be made payable to "Louisiana Vital Records." Cash is not accepted, except if the request is made in person. Fees are not refundable.
