Below is the information you'll need in order to obtain a death certificate in Washington, D.C., including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.
Qualifications
The following people may request death certificates from Washington, D.C.:
- Parent
- Child
- Sibling
- Informant named on death certificate
- Spouse
- Personal representative or executor
Cost
The fee to search for a death certificate is $18.00, which includes one certified copy of the death certificate. Each additional copy of the record ordered at the same timeis also $18.00. Checks or money orders should be made payable to "DC Treasurer." Cash is not accepted. Fees are not refundable.

- Death certificate number (if known)

- Photocopy of current, government-issued photo ID

You may request a certified death certificate in person at the Office of Vital Records, located at:

Washington, DC 20002
The office is open from Monday through Friday from 8:30 am until 3:30 pm, and is closed on major holidays. There is usually a 15-30 minute wait. You must bring adequate proof of your identity.
