Delaware Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in Delaware.
Overview: The Delaware Living Will is called "End-of-Life Decisions" and is included in an Advance Directive packet that also contains a forms for naming a Power of Attorney for Health Care (aka Health Care Proxy) and specifying Anatomical Gifts.
Required Information: You and two (2) witnesses must sign your DE Advance Directive for it to be considered valid Signature. Notarization is optional, but recommended
