Alaska Death Certificate

Below is the information you'll need in order to obtain a death certificate in Alaska, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.

Qualifications

A spouse, parent, child, or sibling of a person who died may obtain a certified copy of the death certificate. Proof of your relation to the person who died is required when submitting your application. Siblings will need to provide a copy of their birth certificate showing parental relationship to the person who died.

A death certificate can also be furnished to a legal representative or to a person who provides documentation showing the death certificate is needed for the determination of property rights. If you are a legal representative (such as an executor), include a letter stating whom you represent and how you are related to the person named on the record.

Cost

The fee to search for a death certificate is $25.00, which includes one certified copy of the death certificate or a "Certificate of Failure to Find." For each additional copy of the certificate ordered at the same time, the fee is $20.00. Checks or Money Orders should be made payable to the State Board of Health. Cash is not accepted, though credit cards are (Visa, MasterCard, Discover). Fees are not refundable.

  • Purpose of the request
  • Your relationship to the deceased
  • Contact phone number
  • Name of the deceased's mother before she was first married
  • Name of the deceased's father
  • Date of birth of the deceased

Submitting Your Request

All requests must include a copy of government-issued picture ID of the person requesting the death certificate. Your signature under the copied ID is also required.
