North Dakota Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in North Dakota.
Overview: The North Dakota Living Will is called "Health Care Instructions" and is included in an Health Care Directive (Advance Directive) packet that also contains a form for naming a "Health Care Agent" (a.k.a. Health Care Proxy) and specifying Anatomical Donation.
Required Information: You and two (2) witnesses must sign your ND Advance Directive for it to be considered valid.
