Nevada Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in Nevada.
Overview: The Nevada Advance Directive includes a Living Will declaration and a form for naming a Durable Power of Attorney for Health Care (also know as a Health Care Proxy), as well as a Durable Power of Attorney for health care decisions for adults with intellectual disabilities.
Required Information: You AND two (2) witnesses must sign your NV Advance Directive for it to be considered valid.
