Massachusetts Death Certificate
Below is the information you'll need in order to obtain a death certificate in Massachusetts, including qualifications, cost, required information, where to submit your application, and a link to the application you'll need to fill out.
Qualifications
There are no restrictions on who may request certified death certificates in Massachusetts.
Cost
If the request for a death certificate is sent in through the mail, the fee to search for a death certificate is $28.00, which includes one certified copy of the death certificate. If the request is made in person, the fee is $18.00 per copy. If you are requesting expedited service via mail, the fee is $37.00 per copy. Checks or money orders should be made payable to "The Commonwealth of Massachusetts." Cash is not accepted. Fees are not refundable.

- A business-size self-addressed envelope
Submitting Your Request
By Mail:
Send the completed application, the appropriate fee, and a business-size self-addressed envelope to:

If you are requesting expedited service, address your envelope to the attention of "Expedited Mail Service."
In Person:
You may request a certified death certificate in person from the Registry of Vital Records and Statistics, located at:

The Registry of Vital Records and Statistics is located next to the Bayside Expo Center, in the Bayside Office Center. The office is open from 8:45 am to 4:45 pm, Monday through Friday.
