Iowa Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in Iowa.
Overview: The Iowa Living Will is called a "Declaration Pertaining To Life Sustaining Procedures" and is included in an Advance Directive packet that also contains a form for naming a Durable Power of Attorney for Health Care Decisions (aka Health Care Proxy).
Required Information: Your signature must be Notarized OR two (2) witnesses must sign your IA Advance Directive for it to be considered valid.
