Alaska Advance Directive Form
Below is the information you'll need in order to obtain an Advance Directive in Alaska.
Overview: The Alaska Living Will is called "Instructions for Healthcare" and is included in an Advance Directive packet that also contains forms for naming a Health Care Proxy and specifying Organ Donation.
Required Information: You and two (2) witnesses must sign your AK Advance Directive for it to be considered valid.
